Insurance Information

We currently accept insurance plans from the following carriers, although it is subject to change at any time, so please verify coverage with the office when making an appointment.

• Medicare Part B
• AARP
• Adventist Health Systems
• AVMED
• Blue Cross/Blue Shield
• Cigna Medicare
• United Healthcare PPO
• Aetna US Healthcare PPO
• Humana PPO
• Tricare

We recommend you call your carrier to verify coverage when appointments are made to confirm deductibles, co-pay amounts, and coverage. For most policies, in order to receive the best coverage, you should make sure Daniel W. McGrane MD is assigned in your policy as the primary care provider and have chosen a plan in which he is in-network.

Our practice does not accept any HMO insurance plans as primary or secondary coverage.  If you have an insurance carrier that we are not in-network for, you may be required to find another provider by your insurance plan or pay out of pocket when seeing Dr. McGrane.

Payments are accepted by check, cash, Visa, Mastercard, and Discover at the time of service.

Provided below are various forms that require completion prior to your visit. Please download the appropriate form as directed by your physician and/or staff by clicking on the name of the form. Please print and bring the completed form(s) with you or click submit at the completion of each form, and our staff will have it printed and ready for your signature for your scheduled visit.
